Workplace Investigations

Our investigator is a senior experienced human resources practitioner with several years’ experience in this field.  He is also a qualified Incident Causal Analysis Methods System (ICAMS) investigator.  ICAMS is an internationally recognised method for the investigation of ’causes’ of safety events and is a ‘no blame’ process.

We offer investigations services into the following:

  • Employment Relationship issues. These investigations look at the breakdown of relationships to determine the cause.  They are a ‘no fault’ investigation looking to identify cause and remedy not blame.
  • Disciplinary investigations.  Disciplinary investigations are undertaken to support the conduct of a disciplinary process.  These types of investigations delve into a situation to determine the facts of an incident for presentation and consideration by the decision maker in a disciplinary process.  These types of investigation may also be undertaken to determine if a disciplinary case exits.  By default, they are not a causal investigation as they consider the actions and or activities of a nominated individual or individuals.
  • Safety Event Investigations. A safety event investigation considers the causes of a safety incident, accident or near miss. They are a causal ‘no blame’ investigation as they are looking at what caused the event not who was at fault. Safety investigations form part of the risk management and/or assurance components of a safety management system.  Safety investigations are important for ‘Officers due diligence’ and continual improvement of a safety management system.  Investigations into near misses present a company with the opportunity to prevent an injury causing event.
